Different clinics need you to bring different items with you to your visit. If you take these thing with you, you should have your bases covered. To be on the safe side, double check what you need to bring at the specific clinic where you make an appointment.
Photo ID: bring an official picture ID like state ID, driver license, passport, school, or work ID.
Proof of Address: take a copy of your lease, a cable or utility (like ConEd or TimeWarner) bill if you have one in your name. Otherwise, take one with your parent’s name on it.
Proof of Income: Some clinics will help you get insurance. You’ll need your bank statement, letter from your employer stating how many hours you typically work and what you earn, or take in a pay stub.
Medications: take a list of the medicines you are taking, or take them with you to your appointment.
